Allow to add note to symptom#1997

Current version of symptom tracking is good, allows you to track timing and intensity. But things dont really fit neatly into the box if it’s not pain simpliciter and with migraine you have a whole spectrum of sensations to account for. Like eyes feel “gritty”, or a sense of pressure or discomfort (but not really pain) behind the eye. Such sensations dont quite get fully expressed unless you go to the general free form text box. But it would be a lot more contextually-intuitive if users can add a note directly to any particular symptom or area of pain (the skull graphic) rather than at the end of the whole entry.

Also, the intensity tracker is only on one scale. eg my current attack was a carry-on from a postdrome (just a pressure discomfort in the left eye) which grew a new head and became its own monster (and became pain scale 4/10). Although i recorded it in the “area of pain” and “symptom”, it doesnt give a full picture of its characteristic when you read it back at the doctor’s office months later. And for contextual comparison, i retrospectively allocated the postdrome phase a pain scale of 1/10 because it’s not technically pain, but in terms of discomfort it caused during the postdrome phase, i actually would have rated it 3/10. I understand it could be more complex to build a scale that “resets” to a different matrix when the nature of the issue changes, so having the notes would allow a more complete narrative to be recorded.
