Maybe a search feature for chats? I would also think adding a way to have certain actions automatically add to your notes (like, clicking the back right of the head would add “Time/Date: Pain in back right of head” or clicking to change pain scale would automatically add “Pain increased to 4” among other things) would help people who can’t find the words or don’t have time.
